Its a well known fact that any mental disorders cannot be cured with medicines. the role of medicines to reduce the symtoms by which the bizarre behaviour of the patients can be brought under control. systematic psychometry to understand the psychopathology and the causative factors and individually tailored psychotherapy will only prove effective both in improving the functional level of the person and at the same time prevent relapse. It high time that the general public are made aware of the use and effectiveness of psychotherapeutic interventions

Psychotherapy with children and their parents often involves play, dramatization (i.e. role-play), and drawing, with a co-constructed narrative from these non-verbal and displaced modes of interacting. Psychotherapy occurs within a structured encounter between a trained therapist and client(s). Purposeful, theoretically based psychotherapy began in the 19th century with psychoanalysis; since then, scores of other approaches have been developed and continue to be created.
    Therapy is generally used in response to a variety of specific or non-specific manifestations of clinically diagnosable and/or existential crises. Treatment of everyday problems is more often referred to as counseling (a distinction originally adopted by Carl Rogers). However, the term counseling is sometimes used interchangeably with "psychotherapy".Rogerian is a form of nondirective counseling or client centered.

    problems.Now that the Tamilnadu Open University is offering a course in Psychotherapy and counselling hundreds of trained professionals are available to render services in Educational,Industrial,medical, health care, social welfare projects. The prison,police depts and family courts could very well utilise their services.Thanks to the Minister for Higher Education, Principal Secretary Mr K Ganesan IAS and the Vice Chancellors Dr M S.Palanichami and Dr Mrs Kalyani Anbuchelvan we were able to launch the course. Mostly matured, elite people with the flair and aptitude for social service join the course.So far we have successfully completed five batches of participants.

    Autism spectrum disorders (ASD) often can be reliably detected by age 3. In some cases even earlier than that.

    Recent studies suggest that kids may eventually be accurately diagnosed by the age of 1 year or even younger. One thing for certain is that if your child shows any of the early warning signs of autism then they should be evaluated by a professional who specializes in autism disorders.

    Parents are typically the first to suspect that something may be wrong. Some children just seem "different" from birth. They may be unresponsive to people or focus intently on one item for a long period of time.

    Others notice changes in their otherwise normal child. An active, happy toddler may suddenly become silent, withdrawn, show self-abusive behavior, or perhaps become indifferent to social overtones. They suspect something may be wrong.

    Research studies have consistently shown that parents are usually right about noticing development problems, even though they may not know exactly what the specific problem is. This proves what parents already know--trust your instincts when it comes to your children.

    The pervasive developmental disorders, or autism spectrum disorders, run the gamut from a severe form, called autistic disorder, to a relatively milder form, Asperger's syndrome.

    If your child has symptoms of these disorders, but does not meet the specific criteria for them, then the diagnosis is called pervasive developmental disorder not otherwise specified (PDD-NOS). Two other rare, very severe disorders that are included in the autism spectrum disorders are Rett syndrome and childhood disintegrative disorder.

    staying at home with parents are important.
    Now because both the parents are working and
    aged parents are left in HOMES, the children
    do not understand the magic of love and affection.The old story of the grand child keeping safely the broken vessel to be used for the child's father is a point to be pondered over.

  30. Are You Depressed?


    Most of us are under the impression that Antidepressants are effective in doing away with depression.
    Recent studies at the University of Hull prove that these pills are useful only for severely depressed people. These studies recommend that the other cases of depression should only go in for counseling.
    It will be of interest to note that all these studies analyzed the clinical trials of the newer SSRI depressants (Selective Serotonin Reuptake Inhibitor), including Prozac and Xeroxtat. SSRIs are the most widely prescribed antidepressants in many countries. SSRIs are believed to affect the chemical behavior of a specific biological target such a hormone receptor may gave therapeutic value.
    Patients with depression were divided into two groups and one group was given the drugs and placebo was administered to the other group. Results proved that only a marginal improvement was noticed in the drug administered group, as compared with the placebo group.
    The study concluded that most patients can benefit as much from other treatments such as physical exercise, and psychotherapy, as they can from medication. The immediate need for the depressed person is human touch and safe environment. No medication could fulfill this need. On the contrary the make the person tired, subdued, and in some cases even hyper active. It is no good giving antidepressants to someone who needs talking therapy. The idea of the therapist should be to respond to the patient’s needs and give them exactly what they want.
    We should pressurize the Government to include Psychotherapy to cover in insurance scheme.

    I think antidepressants are prescribed for anything and everything not only by psychiatrists by also physicians. Antidepressants have many serious side effects and this in turn may be a boon in disguise for the pharmaceutical industry.

  33. DSM is being revised periodically by the APP. There is another called ICD of WHO which also brings its classification.Rubbing against the opposite sex is classified as a disorder. There are people who have the tendency to hoard even if the things areof no use.The problem is they wont share them with others. The PTI man who died had a pile of papers which his better halk thoght useless and disposed. The hoarder might be keeping things which others think useless.
    The DSM V which will be released will describe about it.

  39. WHAT IS BEHAVIOR MODIFICATION


    Behavior modification is a type of treatment which focuses on altering maladaptive behavior, to teach patients more adaptive behavior and to break bad habits. There are a number of applications for behavior modification, and there are also a range of approaches to this type of treatment. It is a good idea to consult with several experts before pursuing behavior modification to determine whether or not it is appropriate for a client, and to learn about different approaches to see if there is one which may be more suitable for a client than another.
    In behavior modification sessions, patients are essentially trained out of maladaptive behavior. It can take numerous sessions, and the approach is usually tailored to the patient. Approaches can be as simple as time outs for a child who acts up in class, or as complex as biofeedback systems which are designed to get patients to stop chewing their nails. Some behavior modification utilizes punishments of various forms, which has attracted criticism, as some people feel that punishment is not effective and can even be harmful.

    Phobias, anxiety disorders, and bad habits can all be treated with this type of therapy. For example, a patient who is afraid of water might undergo systematic desensitization to remove the fear. Bad habits which can be treated with behavior modification can include nail biting, hair chewing, finger sucking, and a variety of other issues. Behavior modification can also be used to treat issues such as bedwetting or acting out.

    Patients with developmental disabilities can sometimes benefit from sessions which teacher adaptive behavior so that they will feel more comfortable in society. Behavior modification is also used to treat conditions such as autism, providing patients with skills which will increase their level of functionality in society. Ongoing behavior modification therapy can be used in both children and adults.

    While the term “behavior modification” may sound a bit ominous, sessions are usually gentle. The practitioner does not want to create additional behavioral issues by approaching the patient aggressively or choosing a method of approach which is inappropriate to the patient. While some tactics in former eras may have been harsh or abusive, most practitioners today recognize that these methods of treatment are not effective and can be actively harmful, preferring the carrot to the stick when it comes to helping theirclients.

    Step I - A: Telling the Story
    • Clients need to tell their stories – that is reveal their Problem Situations and Unused Opportunities. Some clients are verbal, while others may be mute. Some may reveal everything that is bothering them, while others are quite reluctant
    • Counsellors need good communication skills
    • Good relationship building by Counsellor
    • Counsellor has to assess the nature and severity of the problems faced by the client
    • Counsellor has to assess personal, inter-personal and environmental resources of the client
    • Stories may come all at once or in bits and pieces

    Step I - B: Focusing
    • Screening, Focusing and Clarifying
    • Screening: Is the Problem Situation relevant to treat or does it need referral
    • Client reveals number of issues; determine which is primary
    • One issue is identified. Counsellor helps Client to clarify the issue in terms of specific patterns of experiences, behaviors and feelings
    • If required, sometimes Counsellor challenges it
    • In this process Client comes to know clearly what needs to be changed

    Step I - C: New Perspectives
    • Old and comfortable frame of reference keeps Clients locked into self-defeating patterns of thinking and behavior
    • Challenging blind spots – helping Clients see themselves, others and the world around them in more creative ways

    Positive Psychotherapy (PPT) (since 1968) is the name of the method of the psychotherapeutic modality developed by Nossrat Peseschkian and co-workers. Prof. Peseschkian, MD, (1933–2010) was a specialist in neurology, psychiatry, psychotherapy and psychotherapeutic medicine.
    Positive psychotherapy is a method in the field of humanistic and psychodynamic psychotherapy and is based on a positive image of man
    Today positive psychotherapy has been established in more than 33 countries with approximately 30 independent centers and institutions.
    The method of Positive Psychotherapy belongs to the Humanistic, Trans cultural and Psychodynamic Psychotherapeutic methods. The method is based on the new positive image of man that man is good by nature and endowed with four kinds of capabilities: physical, mental, social, and spiritual.
    Illnesses and disorders are therefore interpreted in a new and positive way, and through the use of stories, anecdotes and examples from other cultures the patient is encouraged to play a more active role in his own healing process. Positive Psychotherapy believes that every human being is an entity of body, mind, emotions and spirit.
    The aim of the therapeutic process is to help the client to develop his inborn capabilities and to find the balance in his daily-life. The patient is encouraged to become a therapist for his own family and environment. Elements of other psychotherapeutic methods are used in order to be flexible in the treatment of the patient: Positive Psychotherapy is an interdisciplinary approach."

  91. 1 - The Principle of Hope implies that one does not try to immediately eliminate a disturbance but to first understand it in a broader context and to respond to its positive aspects. The word "positive" (lat. positum) means "the actual", "the given", "really" – the aim of positive psychotherapists is to help their patients to clear the view on the disturbance and recognize their meaning (basic-capabilities and actual-capabilities). Accordingly the disorder will be reinterpreted.
    Some examples:
    • Sleep disturbance is the ability to be watchful and get by with little sleep
    • Depression is the ability to react with deepest emotionality to conflicts
    • Schizophrenia is the ability to live in two worlds or to resort to living in a fantasy world
    • Anorexia nervosa is the ability to get along with few meals and identify with the hunger of the world
    Through this positive view a change of standpoint becomes possible, not only for the patient, but also for his environment. Hence, illnesses have a symbolic function which has to be recognized by both therapist and patient. The patient learns that the symptoms and complaints of the illness are signals to bring his or her four qualities of life into new balance.
    2 - Principle of Balance: Conflict Dynamics and Conflict Contents. The four qualities of life. Primary and Secondary capacities. Despite social and cultural differences and the uniqueness of every human being, it can be observed that during the management of their problems that all humans refer to typical forms of coping.
    Thomas Kornbichler [1] explains: "Nossrat Peseschkian formulated with the Balance Model of Positive Psychotherapy (an innovative contemporary approach to dynamic psychotherapy) a vivid model of coping with conflicts in different cultures."
    According to the balance model, the four areas of life are: 1. body/sense – psychosomatic; 2. achievement/activities - stress factors; 3. contact/environment – depression; 4. fantasy/future/world view/meaning of life - fears and phobia.
    If we get into conflict by stress or micro-trauma, we can express our conflict-situation in four typical forms of the conflict-solution:
    • body-oriented modes
    • achievement-oriented modes
    • relationship-oriented modes
    • fantasy-oriented modes
    Though these four ranges are inherent in all humans, in the western hemisphere the emphasis is more often on the areas of body/senses and profession/achievement in contrast to the eastern hemisphere where the areas are contact, fantasy and future (cross-cultural aspect of positive psychotherapy). Lack of contact and imagination are some of the causes of many psychosomatic diseases. Everybody develops his or her own preferences on how to cope with conflicts that occur. Through a one-sided mode to the conflict solution, the other modes are getting eclipsed. This one-sidedness in the four qualities of life leads to eight typical modes of conflict reaction, depending on an active or passive type of coping:
    • Overcompensation in body-cult (narcissistic over-valuation of the body)
    • Decompensation in somatic illness (somatisation, addictive behaviours, risk factors: overweight etc.)
    • Overcompensation in activity and achievement
    • Decompensation in achievement- and concentration-problems
    • Overcompensation in sociality
    • Decompensation in loneliness
    • Overcompensation in megalomania, delusions, etc.
    • Decompensation in senselessness, existential fears.


    The conflict contents (e.g. punctuality, orderliness, politeness, trust, time, patience) are described in terms of primary and secondary capacities, based on the basic capacities of loving and knowing. This can be seen as a content-wise differentiation of Freud's classical model of the instances.

In the mean time our beloved Professor has contributed very valuable materials. I would like to recall Professor's postings on 19-10-10 at 7.30 A.M ie Why do Gods give trouble to the good people?.     First of all our relationship with God is Father and Sons/Daughters and not God and flies.
    Some times we one may not be unable to relate with Him as Son/Daughter due to earthly character viz sexual immorality, impurity and debauchery; idolatry and witchcraft; hatred, discard, jealously, fits of rage,selfish ambition, dissensions, factions and envy; drunkenness, orgies. and the like. When we are imbibed with celestial character like love, joy, peace, patience, kindness, goodness, faithfulness, gentleness and self-control, we can relate with Him as Sons/Daughters. Then we will be able to say boldly as Professor said on 20-10-10 at 2.36 A.M as 'Things happening are for good'.

    Among the three Cardinal Principles, the Principle of HOPE appears to be the heart of the therapy.
    A contemplation on HOPE.
    When all hope, as a human possibility fails one should put hope in belief. Hope does not disappoint us. Hope is not to be equated with unfounded optimism. On the contrary it is the blessed assurance of our future destiny and is based on unconditional Divine love.
    Hope that is seen is no hope at all.Who hopes for what he already has?. But if we hope for what we do not yet have, we wait for it patiently. Hope is a cause for joy in affliction. Hope can not be conjured up by man's effort; it is God's gift.
    Three great virtues are faith, love and hope. Hope is not wishful thinking but a firm confidence. This unusual thought of faith and love come from hope.

    Thank you for your thought provoking contributions. Now a days, in marital relationship the question why? is very common; that is why we find hard to find role model.
    Vasuki obeyed what ever Thiruvalluvar said. Outwardly this relationship appears to be that of boss and subordinate. But in holy matrimony a process takes place by which they become one in thought, talk and deed. In that sense neither the person saying is boss nor the person obeying is subordinate. Vasuki did every thing happily, whole heartedly and without any fear. She is not an ordinary woman but a woman of Celestial character.
    When Vasuki rushed to attend the call of her husband the rope and the bucket, which she was using to draw water from the well, rested in the middle of the well.This shows her purity and holiness in other words the virtues of faith, love and hope. Therefore Vasuki is to be considered as a noble wife and worth to be emulated by modern women in order to live a harmonious, happy and prosperous life.

  113. I recall someone mentioning about suicide helpline which is in effect (not in tamilnadu) in some other state. Setting up one such line and making people aware of such help is someting to consider.

    Thank you and appreciate your willingness to share your expertise through blogs.

    If a person is experiencing Mental and behaviour challenge, psychologists are offering them Internal Medicines, it does help to quickly recover from severe sufferings but they are experiencing ongoing struggle with day to day activities, their ability to deal with their negative thoughts, emotions, ability to interact with social life, worries and their ability to not able to come out of their habits like smoking.
    Please advice if there is additional treatment can be given, what are the best ones would you recommend besides internal medicine to help to recover.

    1. It s Psychiatrists who prescribe internal medicines like antidepressants which is palliative and of course help the patients to come out.Psychotherapy like Logo Therapy
      making the clients understand the meaning of life,and abilities to cope up with crisis,would help.Intervention is very crucial.

  115. Very timely and contextual. Dr. Frankl confirms that finding a meaning of life, prevents psychological disorders in the individual. If therapies like Logo therapy are made available to the general public (whether the individual has a mental disorder or not) would prevent many psychological or social problems. Consumption of toxic medicines also will be highly reduced.

THE FREQUENCY AND THE NUMBER OF SUICIDES ARE INCREASING EVERYWHERE AND I HAVE BEEN LECTURING ABOUT SUICIDE AND ITndS IMPACT ON BRAIN, BODY AND ENVIRONMENT(Audiovisual power point presentation) The behavioural pattern and the mood of the man trying to commit suicide are unique and if noticed earlier may save the patient.
    I find in " THE HINDU DATED 17TH AUGUST 2012 ABOUT THE MAXIMUM NUMBER OF

    SUCIDE IN NEW YORK 12 in oneday on august 12th 2012. (Quote) " Marilyn Monroes death

    may have been partly responsible for New yorks record 24 hour total of 12 suicides on AUGUST

    12th.Mr John Foley,administrative Assistant to New Yorks chief medical Examiner, said" The

    highest for a day I remember during the ninteen years I have been here was eight.That was

    sometime last year" The average New York suicide rate is two a day.(unquote)



    Suicide rate has increased among the student population and the main factors are:

    1.failure

    2.frustation

    3.fear

    4.fanatism.

    5. financial collapse.



    Of late suspicion and lack of trust among the married couple, honour killing, eloping with the lover after compulsory wedding to an arranged match and victims of rape and abuse.



    A short concise guide note to PSYCOTHERAPISTS, factory managers, armed forces, and family members is appended below.This is part of my audiovisual power point lecture on "suicide and its impact on brain,body and environmrnt".



    THE ONLY WAY TO REDUCE THE INCIDENCE AND PREVENTION OF SUICIDE IS TO

    OBSERVE EVERY ACTION AND ABNORMAL BEHAVIOURAL CHANGES IN THE PERSON DAYS BEFORE ONE COMMITS SUICIDE AND CARE AND ADVISE THE PERSON.

    Message

    Sinceone million commit suicide every year as per WHO, we must preventsuch acts. Person is in painthat he or she can see no other options. Suicide is a desperateattempt to escape suffering that has become unbearable. Blindedby feelings of self loathing, happiness and isolation a suicidalperson can't see anyway of finding relief, except through death. Butdespite their desire for the pain to stop, most suicidal persons aredeeply conflicted about ending their own lives. They wish therewas an alternative to commit suicide but they just cannot see one. Before suicide some clues of warning are evident. Person seekslethal weapons, collects them and speaks well about them, and alwaysboasts of death and executes will in advance. Do not ignoresuicide threats. The feelings of a person who is to commitsuicide, says either casually or jokingly and it means serioussuicidal feelings. We can play a role by finding thealternative showing that you care more for him and promising to get adoctor,psychologists to help. This assurance and help will divert histhoughts. Suicidal frame include dramatic mood swings, suddenpersonality and behavioural changes such as, from out goingwithdrawal or well behaved to rebelious. Finally person may loseinterest in day to day activity and appearance show bigchanges eating and sleeping habits.

  123. As an attempt to prevent various psychological issues, psychotherapists can counsell the parents at different levels.
    we can form an awarness questionare for parents to know how much they know their child and her/his needs. then counsel them.
    Parents need to be counselled at a major scale, we hardly see parents who try to understand children like before, we see mostly self-centred parents, interested in individual happiness, career growth etc.The broader vision "for the sake of next generation " has diminished significantly, sending children to expensive schools ,additional classes and buying them expensive gadgets has become a temporary pride.So children dont yearn for anything, even before they decide if they need it they already have it coz the parents think they are giving their best. Both new and adoloscent parents have to be counselled so that we can be sure of a healthy next generation.
